Temporo Mandibular Joint Disorder ( TMJD)
TMJD related pain affects up to 12% of the population alone and up to 24.8% of the population if Bruxism and Chewing difficulty is included. This condition can be difficult to treat by conventional methods and is often chronic in nature. It is frequently associated with chronic daily headaches and neck pain. TMJ disorders are frequently either caused or aggravated by weakness of postural neck and shoulder girdle muscles.
A thorough examination is conducted on every patient. Structural deficits, restrictions, as well as muscle weakness and imbalances are identified. X-rays and state of the art Ultrasound technology are used for diagnosis of structural jaw abnormalities. Advanced imaging techniques such as MRI can be used for further assessment.
Dextrose and Biocellular Prolotherapy
Ultrasound guided steroid injections of the TMJ joint can relieve pain and inflammation of arthritic joint. Frequently TMJ is a victim of underlying ligamentous instability of the joint and joint meniscus that starts to shift and sublux with chewing. We treat the entire TMJ ligament, tendon, and meniscus complex to promote joint stabilization. Treatment of underlying TMJ instability frequently helps with headaches, facial, and ear pain. Unlike a more commonly used target specific approach where just a joint is injected, our technique restores stability to the areas around the injury. This not only allows for better healing of painful joint but prevents recurrence of breakdown of the area in the future.
Concentrated dextrose solution is the foundation of our regenerative treatments. When injected around the ligaments and tendons of the TMJ, it promotes ligament regeneration and mild cartilage repair. It stimulates and enhances both PRP and Stem cell’s regenerative potential.
PRP (platelet rich plasma) is a stronger biocellular treatment. It promotes joint cartilage regeneration and repair of worn ligament and tendon. PRP Prolotherapy technique is used in more advanced arthritic and degenerative cases. It directly delivers grown factors in the damaged tissue and activates a signaling mechanism that stimulates body’s stem cell production and recruitment to the site of injury. Repair and new tissue growth occurs quickly and naturally.
Osteopathic Manual Therapy (OMT) is utilized during every physician encounter as needed for correction of the jaw alignment and upper cervical joint restrictions that frequently travel with the TMJD.
State of the art personalized one-on-one physical therapy is conducted to address restrictions, correct imbalances and promote strength of the supporting facial and cervical muscles.
Private and small exclusive therapeutic Yoga classes are prescribed for further gentle Cervicothoracic muscles. Yoga also addresses stress related causes of neck and jaw pain through teaching of Mindfulness Based Stress Reduction and breathing techniques.
NOTE: Yoga program is typically instituted after completion of the physical therapy and rehabilitation program, but can be used as a first line treatment in cases where there is problem with generally poor exercise tolerance.
